Germany has 7 AeMCs spread across 5 cities — from the Lufthansa corporate center in Hamburg to the military Luftwaffe facility in Cologne. Prices range from €350 to €500 plus VAT, putting Germany firmly in the European mid-range. Not cheap, not expensive, but with some quirks you need to know about: GOÄ-based billing, EMPIC certificate processing, and 2025 ophthalmology rule changes.
This guide covers all 7 centers with verified pricing, pilot reviews, and practical tips. Whether you're a German cadet starting at Lufthansa Aviation Training, an independent student at a local Flugschule, or a foreign pilot based in Germany.

Practical Reality: 4 Centers for Initials
Of the 7 German AeMCs, only 4 clearly offer initial Class 1 examinations with published pricing: Luftwaffe Cologne, ACG Stuttgart, FLYMED Frankfurt, and LH Group Hamburg. DLR and Charité focus on renewals and airline selection. BAF Bremen has minimal public presence. Most German pilots choose between FLYMED Frankfurt and ACG Stuttgart for their initial.

FLYMED Frankfurt — €450 net (Most Popular)
The go-to AeMC for most civilian pilots in Germany. Exam takes about 3 hours. Dr. Rose is widely praised on pilot forums for professionalism — both for routine exams and pilots with special cases. The process is well-organized with online booking available and lead time of about 7 days.
The complaints: Phone reachability is difficult — they're only available by phone for about 1 hour per day. Email works much better. Some pilots report certificate processing delays of several weeks through the LBA system. The doctors are good; the admin side can be slow.
Best for: Most pilots. Centrally located near Frankfurt Airport (FRA). Good balance of price, professionalism, and experience.
ACG Stuttgart — €400 net (Best Value)
Aeromedical Center Germany (flugmedizin24.de). The cheapest civilian AeMC at €400 net. Online booking, only 5-day lead time — the shortest in Germany. Near Stuttgart Airport (STR).
Best for: Price-conscious pilots, anyone in southern Germany (the only AeMC south of Frankfurt). Munich-based pilots: Stuttgart is your closest option at ~2 hours by car or 45 minutes by ICE.
LH Group Hamburg — €500 net (Corporate)
The Lufthansa Group's corporate AeMC primarily serving LH Group pilots and European Flight Academy cadets. Also available to external pilots. Near Hamburg Airport (HAM). 14-day lead time required — book well ahead.
Best for: Lufthansa cadets and employees (often required by training contract). Northern Germany pilots. External pilots can use it but it's the most expensive German option at €500 net (€595 inc. VAT).
Luftwaffe AeMC Cologne — €350 net (Cheapest)
Military (Bundeswehr/Luftwaffe) facility that also accepts civilian pilots. The cheapest AeMC in Germany at €350 net. However, availability for civilians is limited — military personnel have priority. 21-day lead time, no online booking, institutional military environment.
Best for: Budget-focused pilots who don't mind a military setting and longer booking lead time. Don't expect the customer-service polish of FLYMED or ACG.
DLR Cologne — Renewals from €260 net
The German Aerospace Center (Deutsches Zentrum für Luft- und Raumfahrt). Famous for the DLR BU pilot aptitude test used by airlines. Their aviation medicine department handles renewals and airline selection medicals. Near Cologne/Bonn Airport (CGN). Online booking, 10-day lead time.
Best for: Renewal pilots based in the Cologne/Bonn area. Also the place for airline aptitude testing and research medicals. Not the typical choice for a standard initial.
Charité Berlin — Renewals from €270 net
University hospital setting within the Charité — one of Europe's largest teaching hospitals. Prof. Dr. Helmut Landgraf heads the aviation medicine unit. Performs both EASA and FAA medicals. Near Berlin Airport (BER). Online booking, 7-day lead time.
Best for: Berlin-based pilots for renewals. Also useful if you need both EASA and FAA certificates. The university hospital setting means access to specialist departments for complex referrals.
AeMC-BAF Bremen
Affiliated with the BAF (Bundesstelle für Flugunfalluntersuchung — the German accident investigation bureau). Limited public information available. Near Bremen Airport (BRE). Contact directly for availability and pricing.
How GOÄ Pricing Works
German aviation medical pricing follows the GOÄ (Gebührenordnung für Ärzte) — the official physician fee schedule regulated by law. This is different from most European countries where AeMCs set a flat fee.
What This Means in Practice
Prices quoted by German AeMCs (€350–500) are typical ranges, not fixed prices. Your actual bill depends on which specific tests are performed during your examination. A straightforward healthy applicant pays the lower end. If irregularities are found (ECG anomaly, vision issue, hearing concern), additional GOÄ-coded tests increase the bill. This is not a scam — it's how German medical billing works for all specialties.
Always Add 19% VAT
All quoted AeMC prices in Germany are net (netto). The legally required 19% Mehrwertsteuer is added to the final bill. A €450 quote becomes €535.50. Budget accordingly.
External Specialist Costs
Unlike Spain (where everything is included), German AeMCs may refer you to an external ophthalmologist, ENT specialist, or cardiologist. These external appointments are billed separately. An ophthalmologist visit for the mandatory eye exam can add €100–200 to your total cost.
Certificate Processing (EMPIC)
Germany uses the EMPIC (European Medical Practitioners Information Collaboration) system to transmit examination results to the LBA (Luftfahrt-Bundesamt), which prints the official certificate. This adds processing time compared to countries where the AeMC prints the certificate directly.
Expect 1–7 Business Days
Uncomplicated exam, no limitations: Certificate typically arrives in 1–3 business days.
Glasses wearers (RXO entry): The LBA must approve the ophthalmologist's report before printing the certificate with the RXO limitation. This adds 1–2 days minimum.
OML limitations (VDL, VML, VNL) or referrals: Can take weeks. The AeMC proposes limitations; the LBA reviews and decides. The certificate is only printed after LBA approval.
German AMEs for Renewals
Germany has hundreds of AMEs (flugmedizinische Sachverständige, commonly called Fliegerärzte). For Class 1 renewals, using a local AME is cheaper and more convenient than returning to an AeMC. German AME renewal costs range from approximately €200–400 net depending on scope, billed under the same GOÄ system.
Finding Your AME
The LBA maintains a searchable database of authorized AMEs at lba.de. You can filter by location and qualification level. Ask fellow pilots at your Flugschule or Verein (flying club) for personal recommendations — word of mouth is the most reliable way to find a good Fliegerarzt in Germany.
RTH Pilots: Age 60+ Rule
Rescue helicopter (RTH) pilots must have their first renewal examination after turning 60 at an AeMC — not an AME. Your regular Fliegerarzt cannot conduct this specific exam. Plan ahead.
What Changed in 2025
New ophthalmological requirements took effect in 2025:
Mandatory Ophthalmologist Visits by Age
Class 1 pilots: Ophthalmologist exam at age 45 and age 50.
Class 2 pilots: Ophthalmologist exam at age 50.
All pilots 60+: Contrast sensitivity test required (this can be done at the AeMC/AME, not necessarily an external ophthalmologist).
Should German Pilots Go Abroad?
Germany is mid-range at €420–595 (inc. VAT) for an initial. The question is whether the savings from going to Eastern Europe justify the travel. Here's the math:
| Destination | Medical | Travel | Total | Saving |
|---|---|---|---|---|
| Prague (CZ) | €220 | ~€80 | ~€300 | €120–295 |
| Sofia (BG) | €150 | ~€120 | ~€270 | €150–325 |
| Riga (LV) | €180 | ~€100 | ~€280 | €140–315 |
| Germany (baseline) | €420–595 | — | €420–595 | — |
Savings of €120–325 on the initial are meaningful for students on a tight budget. For pilots in Munich, Prague is a 3.5-hour drive or €20 FlixBus — making ULZ Prague a very practical alternative. Berlin pilots could consider Košice (Slovakia, €341, same-day). For annual renewals, the savings are smaller — a local German AME at €200–280 is more practical unless you're flying anyway.
