Madrid — 4 Centers (€400–€500)
The most AeMCs of any Spanish city. CIMA Madrid (€400) is the cheapest and the government-affiliated center — but pilots report long waits and a 3.5-month certificate delay. The exam itself takes about 6.5 hours including a written personality test and EEG. Servicio Médico Iberia (€420) at Barajas Airport gets excellent reviews for renewals — certificate in hand within 7–10 days — but may not be authorized for initial Class 1 exams.
Hospital Pío XII (€485) and Gabinete Médico Velázquez (€500) are private options with online booking and smoother processes.
Barcelona — 3 Centers (€350–€500)
Barcelona offers the best value in Spain. Centre Sanitari Can Mora (€350) is the cheapest AeMC in the country but only speaks Spanish and Catalan — no English. DiagnosisMedica (€500) speaks English and German. Hospital Quirónsalud (€500) has online booking.
Just outside Barcelona, Sociedad Salut Vallès (€460) speaks five languages — Spanish, English, French, German, and Italian — making it the most multilingual AeMC in Spain.
Valencia — 1 Center (€380)
SERMESA Valencia — second cheapest in Spain at €380. Online booking available. English spoken. A strong option for pilots training at nearby flight schools like FlyBy Spain or Air Griffin.
Bilbao — 1 Center (€425)
Medical Crew Bilbao — online booking, speaks English and French. The only AeMC in the Basque Country and northern Spain. Useful for pilots based at Bilbao Airport or training at schools in the northern corridor.
Sevilla, Málaga, Palma, Tenerife
CMA Triana Sevilla (€560) — speaks Arabic in addition to Spanish and English. Alboran Málaga (€650) is the most expensive Spanish AeMC. Both serve the Andalucía flight training market.
CMA Quirón Prevención Palma de Mallorca — no pricing published but speaks English, German, Swedish, Danish, and Norwegian. Clearly targeting Scandinavian pilots who train or live in Mallorca. Centro Médico La Cuesta Tenerife — Spanish only, no pricing published.
Spain-Specific Rules
Spain applies EASA Part-MED plus additional national requirements. These affect your exam experience.
Mandatory Psychiatrist Interview + Personality Test
Every initial Class 1 in Spain includes a face-to-face psychiatrist interview and a written personality assessment (typically 1–2 hours of written tests plus a clinical interview). This is a Spanish national requirement not found in most other EASA countries. It's included in the quoted price at all centers.
EEG at Some Centers
CIMA Madrid includes an EEG (electroencephalogram) as part of the standard exam. Not all Spanish centers include this — it depends on the center's protocol and the pilot's age/history.
Exam Duration: 4–6 Hours
Due to the additional psychology component, Spanish initial exams take longer than in most countries. Plan for a full day. CIMA Madrid reports approximately 6.5 hours. Arrive early, bring snacks, and clear your schedule.
The AESA Certificate Delay
This is the biggest practical difference between Spain and the rest of Europe. In most EASA countries (Bulgaria, Latvia, UK, Germany), you receive your certificate on the same day or within 1–2 weeks. In Spain, AESA processes all certificates centrally through the Sección de Licencias del Personal.
Expect 30–90 Day Processing
Standard processing time is 30–90 days from the date of examination. At CIMA Madrid, pilots have reported delays of 3.5 months for certificate issuance. During peak periods (September–January), delays tend to be longer.
Impact: If you need your certificate by a specific date (flight school start, airline assessment), add at least 3 months to your timeline. Spain is not the country for a last-minute medical.
Exception: Iberia Medical for Renewals
Servicio Médico Iberia at Madrid-Barajas reportedly delivers renewal certificates in 7–10 days — much faster than other Spanish centers. If renewal speed matters and you're Madrid-based, Iberia is the best choice (€260 renewal).
For Non-Spanish Pilots
Spain is a common medical tourism destination. Flight schools in southern Spain (FlyBy, Quality Fly, CAE Madrid, Aerotec Escuela) attract students from across Europe who then need a local AeMC. Scandinavian pilots who own property in Mallorca or the Costa del Sol also book locally.
Best Centers for Non-Spanish Speakers
Scandinavian pilots: CMA Quirón Prevención Palma — speaks Swedish, Danish, Norwegian, German, and English.
German-speaking pilots: DiagnosisMedica Barcelona or Sociedad Salut Vallès.
French-speaking pilots: Medical Crew Bilbao or Sociedad Salut Vallès.
English-only: Any center except Centre Sanitari Can Mora (Spanish/Catalan only) and Centro Médico La Cuesta Tenerife (Spanish only).
Consider the AESA Delay
If you're coming to Spain specifically for a cheap medical and need the certificate quickly, consider instead: Bulgaria (€150, same day), Latvia (€180, same day), or Czech Republic (€220, 2-day exam but faster certificate). Spain offers better value than Northern Europe but slower certificate issuance than Eastern Europe.
